“The epithelium lining the nasopharynx, middle ear, paranasal sinuses, and larger airways are covered by motile cilia, and the mucociliary escalator is a critical local defense that protects the upper and lower respiratory tracts. Effective airway clearance is dependent on closely coordinated and regulated ciliary function, and any disturbance in the precise, orchestrated movement of cilia can cause disease.

Today’s featured artist is Kathy McRight with her submission titled “Miss Prissy”.

“PCD is a genetic disorder, meaning it is inherited from one’s parents and cannot be acquired from the environment. PCD is most often passed in what is called an ‘autosomal recessive’ pattern of inheritance, in which the disease is only expressed when a child inherits two copies of a mutated gene—one from each parent. The parents are considered ‘carriers’ because they ‘carry’ the mutation, but are not sick themselves because they only have one copy of the mutated gene. When two carriers produce children, however, they have a 1 in 4 (25%) chance of having a child who will inherit both of their mutations, which will result in the disease being expressed.

Lots of different genes have been found to be abnormal in PCD, but we don’t know everything about PCD genetics at this point, and suspicion of PCD requires diligent workup from PCD experts to make sure that the diagnosis has not been made in error.

“To date, there is not a single evidence-based therapeutic intervention for PCD. International efforts are underway to address this situation, but until we have good evidence, we rely on the strong expert consensus of experienced clinicians and researchers to determine the most reasonable therapeutic approaches.

In 2015, the PCD Foundation, in collaboration with the North American PCD research group, published a North American PCD Consensus Statement in Pediatric Pulmonology.  This document represents current best practices as defined by the North American Genetic Disorders of Mucociliary Clearance [Research] Consortium and by the PCD Foundation Clinical and Research Centers Network (PCDF-CRCN).” ~ American PCD Foundation

“The symptoms in patients with PCD are due to defects in cilia that sit atop cells lining the respiratory tract, from the nose to the smallest airways of the lungs. The cilia do not beat rhythmically, making it difficult for mucus to move properly. Bacteria remain in the airway, causing frequent infections in the lungs and sinuses.

Infants with PCD have difficulty breathing and may initially require supplemental oxygen or even breathing support. At some point, their breathing improves, but other symptoms may increase with age such as persistent ear infections, runny nose, and a daily wet cough.

The build-up of mucus can result in bronchiectasis, which causes bronchial tubes to weaken and stretch out, and often leads to pneumonia.” ~ St. Louis Children’s Hospital
