“Bronchiectasis can result from infectious, genetic, immunological and allergic causes. 60–80% cases are idiopathic, but a well-recognised genetic cause is the motile ciliopathy, primary ciliary dyskinesia (PCD). Diagnosis of PCD has management implications including addressing co-morbidities, implementing genetic and fertility counselling and future access to PCD-specific treatments. “Recently, delineation of genotype–phenotype relationships has been emerged in patients with PCD. For example, patients with absent inner dynein arm with microtubular disorganization who harbor a CCDC39 or CCDC40 mutation, have worse lung function compared with patients with outer dynein arm defects and DNAH5 mutation. In contrast, patients with RSPH1 mutations appear to have mild lung function impairment. “Unexplained neonatal respiratory distress is a possible manifestation of PCD. Transient tachypnea of the newborn, starting soon after birth, is a well-known cause of respiratory distress in term and near-term newborns, and resolution by the fifth day of life is generally reported. Conversely, more than 75% of full-term neonates with PCD require continuous supplemental oxygen for days to weeks.
